Your question is very relevant. Stated as simply as possible, the best
answer is:
ELEVATION is measured in meters or feet above MSL - mean sea level and is
a linear measurement. It does not take account of the SL tidal variations
you correctly note. It is really a geogrtaphic convenience.
ALTITUDE is commonly used to describe the physical (and therefoire
physiological) characteristics. We use the two terms interchangeably
unless we want to be precise. Then the best way to define altitude is in
mm HG or torr (these are barometric pressure terms). Milibars are
slightly different and mostly used in aviation.

Most physiologists talk in terms of mmHG or torr in their papers.

To complicate the matter further, the relationship between ELEVATION and
ALTITUDE varies with weather, of course. BUT it is also different at
different latitudes! The physiological altitude near the North Pole is
2000 feet higher than at the equator. This means that altitude illnesses
are more common and more severe on Mt McKinley than at a similar
elevation near the equator (say Africa).
